Cholesterol

How high is too high if the goalposts keep moving?

As a practitioner, you know that cardiovascular disease is a big problem. Regardless of your clinical focus, many of your patients will be dealing with, or worried about, some aspect of their heart health. Without doubt, you will see high cholesterol in your practice and your patients will have questions they look to you to answer.

But to answer confidently you need to:

  • make sense of shifting guidelines
  • navigate conflicting positions that all sound reasonable
  • know what treatments suit the circumstance

It can be frustrating when you know your integrative tools can benefit cholesterol management, but you're not
seeing the results your patients need.

What is getting in the way?

You’re focusing on the short term.

  • We need to successfully reduce cholesterol for more than just a few months at a time. Reducing cardiovascular risk is a big responsibility and impacting future events is a special challenge. High cholesterol is normally symptom-free so knowing you’re managing this risk factor long-term is crucial.

  • We should be playing the long game and setting the scene for future success.

You’re getting overwhelmed by the research.
(It’s like drinking from a firehose!)

  • PubMed has over 30,000 dyslipidemia/hyperlipidemia entries for the last 2 years alone. Yikes. There are multiple expert guidelines, regularly-released position papers, three new lipid-lowering drug classes since 2015, and about a million totally excellent “expert” twitter feeds. It’s a massive amount of information which can feel impossible to translate into clinical practice. And we’re not the only ones feeling it. Patients are under-diagnosed and under-treated across all fields of healthcare.

 You’re using outdated protocols.

  • Lipid knowledge has dramatically increased over the last 20 years. There is clarity where there was supposition. A better handle on risk. More research into integrative therapies and details about dietary strategies. Treatment approaches you learned or researched 10, 5, or even 3 years ago aren’t likely to be sufficient today.

You’re struggling with the contradictions.

  • Despite more evidence than ever before, cholesterol has become a topic of great debate. Seemingly conflicting evidence about causality and treatment has left many integrative practitioners unsure of how to proceed. No one wants to steer their patients wrong.

Knowing what is right will pay off.

Wading through shifting sands

Dr. Alex Verge, ND

For so long, dealing with cholesterol felt like a shell game. I thought I understood how to identify the problem and how to treat it. But thresholds, targets, and guidelines kept changing. The (few) tools I was taught to use didn’t really seem to cut it. With statin therapy dominating the conversation, I even struggled to make sense of how integrative practitioners could make a difference. I’ve flip-flopped on dietary recommendations and suggested niacin reflexively more times than I care to remember.

Patients were coming in confused by their results, frustrated by the lack of direction from their other providers, and left wondering if it even really mattered at all. I often felt unable to adequately answer their questions about what they should do, let alone answer with confidence.

I hated the feeling that patients were falling through the cracks. I worried they would miss an opportunity for change we would both regret down the road.

So I said enough is enough. I needed to know how to answer the questions coming from my patients … and myself.

I wanted to deliver reliable guidance, and results, in an area increasingly troubled with confusion and misinformation.

I started digging and searching and reading.

I got better at explaining lab work and pointing out what might be missing. I became familiar with the pros and cons of risk calculators and how to use them to help direct next steps.

I helped patients develop sustainable lifestyle strategies and encouraged them when they were told drugs were the only solution before even being given the chance to try something else.

But I also learned when to recommend lipid-lowering drugs. And how to complement those drugs for the best outcomes.

And when it felt like I couldn’t always find the information I wanted, I joined the National Lipid Association and started taking courses. I passed the Lipid Competency Certificate examination offered by the Accreditation Council for Clinical Lipidology.

Now I get excited helping people with their high cholesterol!

INTRODUCING:

THE CHOLESTEROL COURSE:
Lipidology Essentials for the Evidence-Based Clinician

In this 10 hour course, you’ll get the background you wished you had every time a patient with high cholesterol walks through your door.

Our course walks you through cholesterol essentials, from top to bottom, so you can answer questions, assess risk, determine needs, choose treatments, support progress, and reach goals.

Cardiovascular diseases (CVDs) are the leading cause of death globally. According to the WHO, almost 18 million people died from CVDs in 2019. 18 million!

Cholesterol is a causal factor for atherosclerotic heart disease and reducing high levels saves lives.

We can make such a difference.

You can make such a difference.

The Cholesterol Course will give you the tools you need to do just that, starting the very next day.

WHAT’S INSIDE YOUR OFFER

Module One - Heart Disease, Cholesterol Causality, and Myth Busting

Our knowledge of lipids and heart health has dramatically increased over the last 20 years. Learn where things currently stand and make sense of all the confusion and misinformation.

You will learn create:

  • The link between cholesterol and atherosclerotic cardiovascular heart disease (ASCVD)
  • Traditional and novel risk factors for ASCVD (including dyslipidemia)
  • Why claims that high cholesterol is safe, or even healthy, aren’t correct

Module Two - Learn about Lipids

We’ll review the biochemistry of lipids and lipoproteins then explore their involvement in the development and progression of atherosclerosis.

You will learn create:

  • Concept of cholesterol years
  • Recognize different dyslipidemia patterns
  • Sex, age, and racial/ethnic difference in lipid levels

Module Three - Testing - Simpler Than You Think

There has been a flood of specialized lipid testing in recent years. Learn why most of them aren’t needed and how to get the information you need.

You will learn create:

  • What lipid tests really matter
  • Which non-lipid tests help in overall assessment
  • The difference between diagnostic and monitoring testing approaches

Module Four - Calculating Risk

Using risk calculators is a critical component of the assessment and management of our dyslipidemia patients. But there are multiple options and there are significant concerns about best use. Learn how to use them appropriately while being aware of their drawbacks.

You will learn create:

  • Compare and contrast various risk calculators
  • Difference between 10-year and lifetime risk calculators

Module Five - Making Sense of Guidelines

Guideline documents are meant to help clarify and streamline decision making. But they are often overwhelming to navigate and necessarily focused on lipid-lowering drugs protocols. Develop confidence using guidelines to help develop treatment recommendations.

You will learn create:

  • Similarities and differences between the 3 main guidelines
  • How to choose and use a guideline as an integrative practitioner

Module Six - Treatment: Lifestyle

Lifestyle management is foundational and where integrative practitioners can really excel. Explore the details and nuances of how to harness the power of these tools in reducing cholesterol.

  • Exercise and ASCVD risk factor reduction
  • Impact of weight management
  • Patient adherence; challenges and monitoring
